Valley View Hospice Los Angeles offers Professional Medication Management (PMM) to help hospice patients use their medications best. PMM means checking medications regularly and ensuring they match conditions, goals, and wishes. PMM also means stopping medicines that are not working, causing problems or are not what you want. PMM can make you feel better, safer, and more comfortable. PMM is done by experts, including doctors, nurses, pharmacists, social workers, and spiritual care providers. They work with you and your family to make sure you get the proper medications for your needs.

What types of services are available?

Our Qualified Caregivers have the experience and compassion to provide assistance with everyday personal care tasks, such as:

  • Expert advice on the appropriate use of medications for symptom management, deprescribing of potentially inappropriate medications, and drug interactions in palliative and hospice care
  • Ensure medication safety by following evidence-based guidelines, using standardized tools and protocols, educating staff and patients, and monitoring and reporting medication errors and adverse events
  • Deprescribing toolkit to help identify and discontinue medications that are no longer beneficial or may cause harm to the patient.
  • Prevention medication diversion and misuse by following recommendations such as securing medications, educating patients and families, documenting medication use and disposal, and collaborating with law enforcement and regulatory agencies
